举报可以口头提出，也可以书面提出，可使用英文或中文。举报人无须使用法律术语或固定格式。如部分信息暂不清楚，可填写"不清楚"或留空。 A report may be made orally or in writing, in English or Chinese. You do not need to use legal terms or a special format. If some information is unknown, write "unknown" or leave it blank.

请尽量写明具体言语、行为、事件顺序，以及受影响人员当时如何回应。不需要使用法律术语。 Include exact words, actions, sequence of events, and how the affected person responded. Do not worry about legal terminology.
请说明被投诉人是否可能继续联系、尾随、报复或对受影响人员及他人造成风险。 Describe whether the accused person may contact, follow, retaliate against, or otherwise create risk for the affected person or others.
