Together with your parent/guardian, please complete your Enrollment Form by Wednesday, December 10, 2025. Be sure to enter information exactly as it appears (or will appear) in your passport and select "Park School | Taiwan Seminar | March 2026".

This form is only required of students under the age of 18 as of March 23, 2026. The Parent/Guardian Waiver Form ensures that CLI has the necessary consent and information to provide a safe and secure learning environment for all our students.
