Pre-Departure To-Do List
Together with your parent/guardian, please complete your Enrollment Form by Thursday, February 15, 2024. Be sure to enter information exactly as it appears (or will appear) in your passport and select "The Blake School | Taiwan Seminar | Summer 2024".
This form is only required of students under the age of 18 as of January 1, 2024. The Parent/Guardian Waiver Form ensures that CLI has the necessary consent and information to provide a safe and secure learning environment for all our students.

This is the information your host family will receive about you! It will also be used by The Blake School and CLI to help pair you with your homestay family in Tainan.
Please take your time completing this form and share as much information with your local host family as you would like.

Note that during our travels in Taiwan, our group will be visiting rural areas and engaging in outdoor activities where exposure to Japanese encephalitis virus (JEV) is possible. According to the Taiwan CDC webpage, JEV transmission intensifies in Taiwan in June and July. Travelers should consider receiving Japanese encephalitis vaccination based on US CDC recommendations.
Your CLI Program Manager will email you a detailed pre-departure reminders PDF one month prior to your journey that includes packing, weather, money, and health and safety considerations.
Be sure to review all information carefully and feel free to contact CLI with any additional questions you may have.
Mandarin is the official language of Taiwan, and a great selection of free Mandarin learning resources is available online.
